SUMMARY: Our thriving medical practice needs an experienced Medical Receptionist who can take over the administrative duties involved with coordinating patient and physician schedules as well as bookkeeping, file management and office organization. We're looking for a professional who believes that patients should be treated as people rather than numbers in a file, and who understands the value of compassionate service. The job includes significant amounts of multi-tasking, but the ideal candidate will know when to slow down, look patients in the eye and provide personalized service.
REQUIREMENRS:
- Welcomes patients and visitors in person or on the telephone, answering or referring inquiries.
- Optimizes patients' satisfaction, provider time, and treatment room utilization by scheduling appointments in a timely fashion with the ability to multitask.
- Keeps patient appointments on schedule by notifying provider of patient's arrival; reviewing service delivery compared to schedule; reminding provider of service delays.
- Call patients to remind them of upcoming appointments and to help them schedule testing for off site services
- Provide patients with intake and new patient forms as well as copies of our office policies and legally required documents
- Ensures availability of treatment information by filing and retrieving patient records.
- Maintains patient accounts by obtaining, recording, and updating personal and financial information.
- Obtains revenue by recording and updating financial information; recording and collecting patient charges; controlling credit extended to patients; filing, collecting, and expediting third-party claims.
- Maintains office inventory and equipment by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; scheduling equipment service and repairs.
- Notifies medical assistants and other clinical staff when important shipments such as vaccines arrive.
- Protects patients' rights by maintaining confidentiality of personal and financial information.
- Contributes to team effort by accomplishing related results as needed.

LOCATION: Macomb County Health Department - Clinton Township, MI 48036
POSITION SUMMARY: Under the supervision of the WIC Program Manager and under the direction of the WIC Breastfeeding Coordinator, this paraprofessional support person will provide basic breastfeeding education and encouragement to WIC families at various Macomb County locations, to help them meet their breastfeeding goals.
ESSENTIAL FUNCTIONS:
- Counsels and educates pregnant and breastfeeding women enrolled in the WIC Program by telephone, virtually or in person, from the WIC clinic or approved alternate work site.
- Maintains a caseload of clients who request breastfeeding information and assistance, and makes routine periodic contacts
- Provides close contact and follow-up to clients during the early post-partum period
- Refers complex breastfeeding issues or problems which are beyond the scope of the peer counselor’s knowledge or skills to the WIC Breastfeeding Coordinator, or alternate breastfeeding professional or health care provider
- Assists in teaching breastfeeding classes or support groups to WIC clients
- Teaches use, cleaning and assembly of breast pumps; expression and storage of human milk; and participates in the breast pump loan and issuance program in the WIC program
- Enters client contact data and education in the MI-WIC system; may schedule appointments and issue benefits when appropriate
- Participates in WIC staff meetings and communicates information to WIC staff as needed
- Participates in breastfeeding training, workshops and conferences as required and thereafter as appropriate
- Bends, stoops and reaches in order to retrieve documents, educational materials, and breastfeeding supplies
- Lifts, moves and carries equipment and supplies that weigh up to 25 pounds
- Be available outside normal business hours.
JOB QUALIFICATIONS:
- Possess a valid Michigan driver’s license and a registered and insured automobile for transportation as needed

Summary: The medical practice of Wayne County Healthy Communities needs an experienced Community Health Worker who is a part of an interdisciplinary clinical care team at a health center who helps increase access to health services for people through outreach, encouraging engagement, education, and peer support.
Requirements:
- Review monthly outreach list for each covered Health Plan and develop a plan for prioritized outreach following MCHN best practices workflow.
- Document all outreach attempts in documentation tool and provide results monthly to allow for aggregate reporting to Health Plans.
- Provide direct education or engagement with contacted patients or connect to other resources for education as appropriate based upon initial assessment.
- Assist in maintaining assigned patient panels with covered Health Plans including helping patients change their primary care provider and supporting the Center in removing patients that are not served by the Center from its panels.
- Assist contacted members with completion of assessment and address triggered interventions.
- Contact assigned Health Plan members identified as either high or inappropriate utilizers of the emergency department and encourage engagement with PCP.
- Utilize reports to prioritize patient outreach attempts to close gaps in care (e.g. missing preventive service, poor chronic disease outcome etc.).
- Facilitate scheduling of required visit / testing and eliminate barriers to care as appropriate.
- Conduct SDOH screening and assessment using tools identified to uncover individuals' needs and connect members with available community resources addressing, but not limited to, the SDOH.
- Report the appropriate detailed documentation utilizing the patient tracking report.
- Attend community outreach events as determined by management to promote patient/community health and education.
- Participate actively in regular supervisory and team meetings and training sessions.

POSITION SUMMARY: This employee is responsible for assisting the WIC registered dietitian in the Wayne County WIC offices by determining client eligibility. The employee will document appropriately; maintain client confidentiality; maintain client records and reports and work collaboratively within the WIC program and other community projects. The employee will complete all training requirements some of which may include state travel.
ESSENTIAL FUNCTIONS: (Performs duties that may include)
- Maintain confidential protected health information according to Department policy and Procedure.

- Perform anthropometric and hematological assessments for applicants.

- Maintain community outreach with area hospitals to promote and support best practices in breastfeeding.

- Demonstrate set-up and break down of multiple types of breast pumps.
- Attend and participate in monthly meetings.
- Maintain contact via telephone with pregnant and breastfeeding women. Breastfeeding Peer educators see women in the WIC Clinics and area hospitals and also have contact by telephone.
- Conduct breastfeeding classes via Zoom and/or other video platforms as well as in person as needed.
- Document all contacts with the pregnant and/or breastfeeding woman within the MIWIC data base according to standards.
- Completes and maintains monthly reports both written and electronic as required.
- Meets all document deadlines as required.
- Identify situations outside the Scope of Practice and refer as appropriate in a timely manner.
- May require travel to multiple offices and sites.
