Kids' Health Connections
KHC helps parents and their children get the most out of their relationship with their family physician by bridging gaps and identifying viable healthcare solutions. KHC serves children in Wayne, Oakland, and Macomb counties.
Kids’ Health Connections provides health education to families, helps doctors improve the quality of their care, and acts as a voice for issues affecting children’s health.
KHC strives to improve quality, access, and health outcomes for children eligible for Medicaid by promoting the Medical Home Model.
Program Goals
- To serve as a Child Health Access Program (CHAP) for the counties of Wayne, Oakland, and Macomb in southeastern Michigan.
- To link families to their doctors & health care providers, Medicaid Health Plans, and community partners to make sure children have the best possible health and healthcare.
- To provide support to pediatric offices and health care providers with trained community health workers and health educators who refer families for services.
Enhances asthma knowledge, asthma control, and quality of life by engaging with specialized asthma educators.
Addresses the social and emotional needs of families, particularly those affected by trauma.
