Kids' Health Connections

KHC helps parents and their children get the most out of their relationship with their family physician by bridging gaps and identifying viable healthcare solutions. KHC serves children in Wayne, Oakland, and Macomb counties. 

Kids’ Health Connections provides health education to families, helps doctors improve the quality of their care, and acts as a voice for issues affecting children’s health. 

Mission

KHC strives to improve quality, access, and health outcomes for children eligible for Medicaid by promoting the Medical Home Model.

Program Goals

Childhood Obesity Intervention
Educates children and their families about nutrition, physical activity, and behavioral health topics.
In-home Asthma Care

Enhances asthma knowledge, asthma control, and quality of life by engaging with specialized asthma educators.

Behavioral and Mental Health

Addresses the social and emotional needs of families, particularly those affected by trauma.

Primary Care Medical Home Support
Seeks to bridge the gap between children, families, and their primary care offices.
