Pragmatic Partnerships
SEMHA Regional Health Collaborative (RHC) is part of a statewide collaboration supported by funding from the Center for Medicare and Medicaid Services (CMS) in collaboration with the Center for Health and Research Transformation (CHRT) and Michigan Medicine. It is administered by the Michigan Department of Health and Human Services (MDHHS).
To date, multiple Regional Collaboratives in Genesee, Jackson, Kent, Livingston/Washtenaw and Muskegon counties are supporting clinical practices in collecting social needs data for their patients and systems for seamless referral and collaboration across community and social service agencies. Each collaborative has worked to align priorities across health and community organizations, by defining regional health priorities, supporting regional planning, increasing awareness of community-based services, and increasing clinical-community linkages to address social needs for individuals with the highest likelihood of experiencing poor health.
The Southeastern Michigan Health Association (SEMHA) has been included as a backbone organization for a new Regional Collaborative in Southeast Michigan and we invite you to join us in this work.
The SEMHA Regional Health Collaborative (SRHC) is convened to advance collaborative efforts aimed at addressing health inequities with a particular focus on Social Determinants of Health (SDOH). SRHC serves as a platform for key stakeholders to share resources, discuss policy, and develop joint strategies to promote equitable health and wellness outcomes in the City of Detroit, Wayne County, and Southeastern Michigan.
The SRHC aims to influence systemic change while empowering communities and creating sustainable health and wellness equity initiatives. We share a collective commitment to reducing health disparities through strategic collaboration, policy advocacy, and equitable resource distribution.

Community Impact
The RHC provided Storyteller Workshops for Community Voice partners to learn how to share their stories and amplify their voices for the benefit of themselves and their communities. Creating a series of videos that demonstrate their vibrancy and resilience while showcasing their lived experiences through their stories.

Covenant House Michigan
Covenant House Michigan’s (CHMI) mission is to end homelessness as we know it by providing young people, ages 18-24, who are experiencing homelessness with emergency housing / shelter, transitional housing, educational and vocational programs, and myriad life skills and wraparound services to achieve a future of stability, pride, and independence. We build a bridge to hope for youth who do not have reliable housing or are survivors of human trafficking by fostering an environment of unconditional love, absolute respect, and relentless support.
Cody Rouge Community Action Alliance
Cody Rouge Community Action Alliance is a community-based development organization (CBDO), and as such, has incredible reach into the community. CRCAA has experienced tremendous success in engaging youth through immersive programming, stabilizing the neighborhoods, and engaging the community through convenings and activation of community efforts. Because of its in-depth engagement processes, CRCAA functions as a neighborhood convener and facilitator, encouraging residents, youth, families, business owners, and churches to come together to solve problems.
COTS
COTS’ mission is to create and facilitate opportunities for families in poverty to collaborate, thrive and succeed in building strong households, neighborhoods and communities.
Founded in 1982 in response to Detroit’s growing homelessness crisis, what began as the Coalition on Temporary Shelter has evolved into a cornerstone of hope and transformation for families experiencing poverty. Now known simply as COTS —the organization has grown into a leading provider of emergency shelter, supportive housing, and family-centered services. Every year we support hundreds of adults and children through our housing programs and transformative coaching framework called Passport to Self-Sufficiency®
Detroit Community Care Network
Detroit Community Care Network (DCCN) is a dedicated Faith-Based Initiative committed to enhancing the health and well-being of residents across Metro Detroit. Established in 2018, today DCCN’s partnership spans twelve local churches and several community based organizations who provide the trained workforce of Community Health Workers and Transition to Success coaches to execute our work. DCCN’s goal is to build a robust, community-centered system of support that empowers individuals and families to live their best lives. By connecting people to essential resources for their social, physical, and mental health needs, we aim to foster resilience, promote self-sufficiency, and create a healthier, more equitable Detroit community.
Detroit Parent Network
Detroit Parent Network (DPN) is Michigan’s premier parent engagement organization, rooted in the belief that every child deserves a champion. We are a dynamic nonprofit network of over 5,000 parents primarily serving low-income families, working together to uplift, educate, and empower.
Founded in 2002, DPN has grown into a trusted leader in Southeast Michigan, bridging families, schools, and systems to ensure children thrive from cradle to career
Detroit Recovery Project Inc.
Detroit Recovery Project Incorporated (DRP) supports communities grappling with drug and alcohol addiction, as well as other marginalized groups. We are committed to helping individuals overcome obstacles to a healthy, productive, and drug-free life. Founded through a collaboration between the City of Detroit Bureau of Substance Prevention, Treatment and Recovery, and the Partnership for a Drug-Free Detroit, DRP has been a nonprofit entity since 2005. Leveraging partnerships at various governmental levels, we offer a wide array of substance abuse prevention, treatment, and recovery services to those exiting treatment and resources to those in long-term recovery.
Detroit Public Schools Community District
Detroit Public Schools Community District is Michigan’s largest public education system. It is governed by a locally-elected, seven-member board with Dr. Nikolai Vitti serving as superintendent. We champion a whole child approach that combines superior academics with socio-emotional, extracurricular, and health services tailored to each student’s needs.
Detroit Wayne Integrated Health Network
The Detroit Wayne Integrated Health Network (DWIHN) is compassionate and committed to the 123,000 children and adults it serves in Detroit and Wayne County. We understand the immense responsibility we have been given and we go above and beyond in supporting the people we serve and the hundreds of Providers in our System of Care. Our Provider Network supports and serves individuals with serious mental illness, children with serios emotional disturbance, people with autism, individuals with intellectual and developmental disabilites and those with substance use disorder.

Molina Healthcare of Michigan, Inc. provides governement-funded, quality health care, serving members through Medicaid and Medicare programs in Michigan. Through its locally operated health plans, Molina Healthcare Inc., a Fortune 500 company, provides managed health care services under the Medicaid and Medicare programs.
Wayne State University Population Health Outcomes Information Exchange
PHOENIX is a data-driven, interactive mapping platform developed by Wayne State University (WSU) to support population health monitoring and improvement. This innovative tool integrates clinical data with social determinants of health (SDOH) to enable comprehensive analysis of public health outcomes across communities.
Designed to support needs assessments, resource allocation, and targeted interventions, PHOENIX provides actionable insights by visualizing and analyzing data at both zip code and census tract levels.
Western Wayne Family Health Centers
Western Wayne Family Health Centers (WWFHC) is your first choice in healthcare for the whole family. Offering primary care for adults, pediatrics, women’s health, Obstetrics, dental and behavioral health care WWFHC can take care of everyone’s health and wellness in one place. Community Health Workers are available to help patients learn more about management of chronic conditions, stopping smoking, preventing HIV, find community resources and to be well.
With locations in Inkster, Taylor, Lincoln Park and Dearborn, getting the best care is easy. Medicaid and most other health/dental insurance is accepted, and there is a sliding fee scale based on income for all services for people who don’t have insurance. The cost of getting care at WWFHC can be quite reasonable.
